A Mexican acta de nacimiento is issued by the Registro Civil of a particular state, and each state formats its certificates differently. What must appear in the English version, without exception: the issuing Registro Civil office and state, the CURP identifier, the book, folio and act numbers, and any marginal annotations — corrections, recognitions of paternity, later amendments. Those annotations are frequently the most legally significant part of the document and frequently the part left out.

The two-surname convention applies throughout. A translation that treats the maternal surname as a middle name creates a mismatch against the passport and the USCIS forms.
